What counts as a small word
Short joining words stay lowercase in the middle of a title: a, an, and, as, at, but, by, for, in, of, on, or, the, to, up. Everything else gets a capital. The first word is always capitalised, even when it is one of those small words, which is why "The Great Gatsby" keeps its capital T.
Title case is not the same as capitalising every word
Capitalising every word gives you "A Tale Of Two Cities", with a capital O on "of". Title case gives you "A Tale of Two Cities". Publishers and style guides use the second one. If you want every word capitalised anyway, use the Capitalize Each Word page.
