Short joining words stay lowercase in the middle of a title: a, an, and, as, at, but, by, for, in, of, on, or, the, to, up. Everything else gets a capital. The first word is always capitalized, even when it is one of those small words, which is why "The Great Gatsby" keeps its capital T.
Capitalizing every word gives you "A Tale Of Two Cities", with a capital O on "of". Title case gives you "A Tale of Two Cities". Publishers and style guides use the second one. If you want every word capitalized anyway, use the Capitalize Each Word page.
